Notice of address change

1. This notice is for you as a vendor, contractor, consultant or lessor/landlord (“you”, “your”) of TB HIV Care NPC (“we”, “us” or “our”).
2. Kindly take note that effective from 1 May 2026, our registered address and principal place of business will change from
   7th floor, 11 Adderley Street, Cape Town 8001 to: UNIT 17-102, Waverley Business Park, Mowbray, Cape Town, W. Cape, 7925
3. We kindly request that you ensure that:
   • Your records, systems and documentation are accordingly updated (for example, statements, legal notices and deliveries); and
   • All invoices reflect the new physical address from the effective date (1 May 2026).
4. This notice serves as a formal notice and the parties herein agree that it will be deemed to amend the relevant provisions of the applicable agreement without the need for any further act, signature, or agreement between the parties. All other terms and conditions of the applicable agreement will remain unchanged and of full force and effect. THC will not be responsible for any breach, loss, damages or claims that occur due to your failure to update your records, systems and documentation.

Notification of a security compromise in terms of the Protection of Personal Information Act, 4 of 2013 (“POPIA”)

Data Security Incident, update as at 14 August 2025,

TB HIV Care (THC) has identified and contained a data security incident that occurred on 28 July 2025. The matter arose from an internal error during routine information processing, resulting in certain employee personal information being unintentionally shared with an unauthorised recipient.

The incident was identified quickly, and immediate steps were taken to contain it within an hour of being reported and to secure the affected systems. At this stage, there is no indication of malicious intent.

In accordance with the Protection of Personal Information Act, 4 of 2013 (POPIA), THC is in the process of notifying all affected individuals. The Information Regulator has been informed. A dedicated support channel has been established to address queries and assist those affected.

We wish to notify staff currently and previously employed by THC in the period January 2022 – December 2023 to contact our offices as this incident is related to information from their period of employment. Contact our Human Resources Department as follows:
